If you wish to be vaccinated, please register to attend one of the upcoming H1N1 clinics by email and/or telephone message to us.

*Please NOTE:
After you email or call in your information to the Bethel Health Department, you are automatically confirmed in the clinic on that date and will NOT receive further follow-up information. Consider yourself registered once you have left your information and the date in which you have registered to attend.

In the subject line type 'H1N1 FLU CLINIC' and the date you wish to attend an H1N1 clinic and include the following information in the email:

        • First and last name of each individual to be vaccinated.
        • Age of each individual
        • If the individual is healthy or has a medical condition.
        • A phone number where you can be reached on the first call
        • Whether they are getting a 1^st or 2^nd dose.
           (Children ages 6 months to 9 years require 2 doses.)
